Programs
HealthReach along with its community partners provides the medical, pharmaceutical, case management and social service programming needed to fully provide for our clients’ health and wellness needs. Our 54 volunteer physicians and medical / pharmaceutical professionals are the cornerstone to the success of our not for profit enterprise. In conjunction with our 14 paid staff these community servants and advocates provide for the diverse medical needs of over 4,500 individuals.
Medical Program – The HealthReach medical program is responsible for delivering The following on-site services:
- General family medicine
- Pediatrics
- Cardiology
- Endocrinology
- Gynecology
- Ophthalmology
- Orthopedics
- Neurology
- Urology
- Laboratory services and phlebotomy
Dental Program – The HealthReach dental program is responsible for delivering on-site oral hygiene and remedial dentistry services. These services are provided in our Mundelein clinic.
Pharmacy Program – The HealthReach pharmacy program is responsible for safely and efficiently filling the prescriptions prescribed by HealthReach physicians and independent referral service providers on behalf of our clients.
Case Management Program – The HealthReach case management program is designed to provide the following services for our clients:
- Patient intake and services counseling
- Application for public aid guidance
- Referral specialty services coordination
- Medical interpreting services both in clinic and out of clinic
HABLA Program – The HealthReach Healthcare Access by Language Advocacy (HABLA) program is a jobs training program designed to educate and certify bilingual English and Spanish medical interpreters for successful careers in the medical industry.
