 HealthReach is in its 19th year of operations serving the medically uninsured low income individuals and families of Lake County, Illinois. We operate free clinics in Waukegan and Mundelein and a licensed pharmacy in Libertyville. HealthReach is committed to improving the physical health of Lake County's low income, medically underserved by providing access to quality medical care regardless of age, race, or origin. We currently serve over 4,500 individuals including men, women, boys and girls of all ages providing a broad range of medical and pharmaceutical services to our clients. With current community trends continuing and new services being brought on line, HealthReach will conduct over 18,000 patient visits and fill over 30,000 prescriptions in 2011.
 
First and foremost, we are a volunteer centric organization of physicians, nurses, pharmacists and other medical and business professionals dedicated to providing a medical home to the less fortunate our community. Our all volunteer Board of Directors is made up of community and industry leaders who are actively engaged in the oversight and funding of the enterprise and provides critical operating support to the Executive Director and paid Staff of the corporation. We are a healthcare safety net provider founded with the mission of providing access to free medical, dental, vision, and pharmacy services to the medically uninsured, low income resident of Lake County, IL. Our objective is to provide a comprehensive medical home that results in improved patient wellness leading to more productive employees, successful students, and healthier families.  
HealthReach and its community service partners provides free access to a comprehensive set of medical, dental, vision and pharmaceutical programs for our clients. Our role in the community is to organize, leverage and apply institutional, human, financial and material resources to ensure the healthcare safety net remains intact for our clients. We focus first and foremost on medical home preventive healthcare for all ages. We also provide on-site access to a variety of medical specialists and laboratory services and we consider the diagnosis and treatment of serious chronic disease such as diabetes, hypertension and cardiovascular disease a core competency.
