We offer primary care medical visits, dental, and vision care for qualifying Lake County, IL residents. HealthReach is a free clinic serving the medically uninsured, low income of Lake County. All our patients must have an income level of no more than 150% ($16,335.00 for a single person, $22,065.00 for 2 in the family and up in increments with the number of family members) above the federal poverty line and provide the information below:
**Medicare eligible persons may qualify for HealthReach services if denied by the Illinois state programs which help to purchase Medicare Parts B and D and the income level meets HealthReach qualifications.
The most recent check stubs, unemployment stubs, or any other form of income including foodstamps and township assistance

Proof of Lake County residency (electric, phone bill, etc.)
