Services
The
HealthReach Community Care Clinic was established in 1992 to provide
health care to uninsured individuals living below the federal
poverty line. We provide access to a continuum of care; from primary
care and simple testing at the clinic, consultation and complex
testing provided through agreements with local doctors and other
providers, to surgery and hospitalization.

Cost
We
are able to provide care for free by utilizing volunteer healthcare
providers. A $5 donation is asked for, but not required, at each
patient visit.

HealthReach
HABLA (Healthcare Access By Language Advocacy)

HABLA
(Healthcare Access By Language Advocacy) is a training and
service program that provides training in medical interpretation
and medical interpretation services to physician offices
and clinics. HABLA is a community outreach effort of HealthReach
Community Care Clinic. No such program exists in Lake County,
IL at the present time. The need for such a program is evidenced
by the impressive growth in the recent immigrant population
enumerated by the Y2000 census, as well as the large undocumented
immigrant population in Lake County.
Training:
- HABLA
will provide training for bilingual/bicultural individuals
interested in volunteering as trained medical interpreters,
those who seek employment as medical interpreters, and
those who seek to enhance their present job skills.
- HABLA
will also provide educational sessions for healthcare
providers and their staff regarding culturally and linguistically
appropriate healthcare, for the benefit of their patients.
Service:
- As
trained individuals become available, arrangements can
be made to use their services in area physician offices
and clinics.
- Consultative
services for physician offices regarding the Office of
Civil Rights Culturally and Linguistically Appropriate
(Healthcare) Services guidelines will be available from
HABLA staff.

Med-Assist
Program-Pharmaceutical Company Indigent Medicine Program Application

Many
companies that make medicine have special programs to help
people who can't afford to buy the medicines they need (called
indigent medicine programs). Each company has its own program
with its special requirements, forms, and procedures. Some
companies have different forms for different medications.
Med-Assist is designed to help uninsured and underinsured
individuals, emphasizing those not patients at HealthReach,
with applications to pharmaceutical company indigent medicine
programs. Med- Assist is an outreach program of HealthReach
Community Care Clinic.
Eligibility:
- Patient
has no insurance coverage for prescription medicine, and
- Does
not qualify for government program that covers prescription
medicines, e.g. Medicaid.
- Patient's
income is low enough that buying medicines causes hardship.
Applicants' income must be below limits set by the particular
medicine manufacturer. The household income limit varies
with each company, from only those below the federal poverty
line with some up to twice that limit.
- Each
pharmaceutical company has final say as to who is eligible
for medications through their programs.

Note:
- Application
help is by appointment only.
- Not
all medicines are available through these indigent medicine
programs.
- Not
all individuals are eligible for these programs; eligibility
is determined on an individual basis by the pharmaceutical
company.
Fees:
- We
request a $5 donation for each application done for an
individual.
- Some
pharmaceutical companies require a $10 Money Order to
cover postage. Patients are responsible for that fee.
Assisted
individuals must provide:
- Name
and address;
- Proof
of income- recent income tax form, 2 recent pay stubs,
or letters and notices that document income (disability,
Social Security, etc.);
- Rx
from a doctor with refills marked, (medications are dispensed
in 3-month portions); and
- Some
indigent medicine programs ask for Social Security number.
