Mission

HealthReach Community Care Clinic is a nonprofit effort in Lake County, Illinois that increases access to primary care for the medically underserved. It provides direct medical care and referral, health education, and information to improve the health status of community members who are most at risk.

Vision

HealthReach Clinic respects the dignity of each individual who comes for care. We try to empower our clients to exercise prudent health and wellness choices while decreasing morbidity and mortality of community members and providing opportunities for community involvement.
